If a child has two homes, that's not a problem at all: they're added to both, and each home fully controls its own routines, with no insight into the other home.

About family & groups
No. You can create a “child profile” without login directly from the Family page — handy for the youngest children. A login can be added later if needed.
The child is added to both homes' groups. Each home creates and manages its own routines for the child, and one home can't see what's happening in the other.
